These forms allow our engineers to provide you the best configuration of our systems for your application. Please be as detailed and accurate as possible and return completed questionnaires to [email protected]. There is a $150.00 fee for engineering time for this service.

Submit Design Form
Form Tutorial
- Each form fee is $150.00.
- Click on the “Choose File” button on the left section (Desktop) or above section (Mobile).
- Choose the local file containing your completed Power Move Custom Design Form or Windshield Wiper System Custom Design Form.
- Enter payment information and complete $150.00 payment.
- A receipt will be sent to you, detailing your completed custom request
Note: Processing time for requests depend on the readability and completion of the submitted form. Please expect longer wait times if submitted form is not complete.
